Нумуларно главоболие – клиничен случай

Нумуларното главоболие (НГ) е рядко срещано заболяване, което може да протича напълно бенигнено или да е изява на подлежащ проблем. Едни от най-основните причини за появата на НГ са травма на главата, аневризма на повърхностен артериален съд, структурна или дерматологична лезия на това място. НГ се характеризира с продължителна или интемитентна болка в главата (най-често париеталната област) с остро очертани граници, фиксиран размер (средна от 1 см до 6 см) и кръгла или овална форма. Главоболието може да се придружава от алодиния, хипо/хиперестезия, парестезия, хипералгезия и повишена чувствителност на мястото. Провеждането на образно изследване е ключово за поставянето на диагнозата. Клиничният случай от нашата клиника е на пациентка с НГ вследствие на травматичен удар преди повече от 30 години на същото място на главоболието, което през последните 3 месеца става ежедневно. Проведе се лечение с pregabaline с титриране на дозата до 150 mg с ефект. При недостатъчно повлияване на симтомите може да се започне лечение с ботулинов токсин.
